Liability Waiver and Release Form

Parent/Guardian Agreement

I, the undersigned, acknowledge that I am the parent or legal guardian of the child(ren) listed below and that I voluntarily seek to allow my child(ren) to participate in activities at Cup of Jo Play Center. I understand that while the facility strives to provide a safe environment, participation in physical activities, including but not limited to play structures, climbing, and social interaction, involves inherent risks.

Assumption of Risk

I acknowledge and accept that there are potential risks associated with the use of the play equipment and facilities, including but not limited to minor injuries (such as bruises and scrapes) and more serious injuries (such as fractures, concussions, or other bodily harm). By signing this waiver, I assume full responsibility for any risks, injuries, or damages incurred by my child(ren) while participating in activities at Cup of Jo Play Center.

Release of Liability

I hereby release, waive, and discharge Cup of Jo Play Center, its owners, employees, agents, and representatives from any and all claims, liabilities, damages, or expenses arising from or related to any injury, loss, or harm occurring at the facility, whether caused by negligence or otherwise.

Supervision and Conduct

I acknowledge that I am responsible for supervising my child(ren) at all times while at the facility. I agree to follow all posted rules and guidelines and to ensure that my child(ren) adhere to them as well. I understand that Cup of Jo Play Center reserves the right to remove any individual from the premises for unsafe behavior, misconduct, or failure to comply with facility policies.

Health & Safety Acknowledgment

I certify that my child(ren) is/are in good health and has/have no medical conditions that would prevent participation in physical activities. I understand that Cup of Jo Play Center is not responsible for providing medical care in the event of an injury and that I am responsible for any medical expenses incurred.
